Registration Information
Please fill out the 3 forms and return to UGA by March XXX, 2011. We only need 1 copy of the "Registration Form" and the "Attendee List". But, we need EACH student and faculty advisor attending to complete the "Release, Wavier, & Indemnity Agreement" form. For this form, you can make a master with the school name and then ask each student to add their personal information and date it. Students over 18 years old do NOT need to include the "under 18 page". Please send your Registration fees with this information.
